- Referral for ABA services from a Qualified Health Professional (QHP) – Prescription for ABA services
- Diagnostic Evaluation Report from a Qualified Health Professional (QHP) – Autism or Related Disorders
- Insurance Card – Front and Back copy
- BB-ABA- Initial Intake Form – Sent by administrator via Dropbox Sign
- BB-ABA – Service Agreement Form – Sent by administrator via Dropbox Sign
Waitlist Status:
Balanced Behavior ABA, PLLC. currently holds a waitlist for direct services.
Parent consultation-only services may be available for individuals seeking immediate support. This is dependent on insurance approval.

We give priority of space to individuals who have submitted all documentation (see above). Once all required documents are received, a confirmation of file completion will be provided, at which time you will also receive your current spot on the list.
Time estimates may vary based on staff/client schedule availability, service format, and access to appropriate training/support for our staff.
